  • Skilled Nursing

    Skilled nursing services are for residents that require 24-hour care by a team of medical professionals such as nurses, nurse assistants, therapists, and physicians who make rounds within the facility. Residents are often recovering from a hospital stay, participating in a therapy-based rehabilitation program, or managing a chronic illness. Skilled nursing services include wound care, cardiac care, respiratory and pulmonary care, stroke recovery, and neurological care.

  • Rehabilitation & Therapy

    Rehabilitation encompasses a broad range of services that facilitate the safe transition of our guests from a post-acute care hospital stay back to their home. Services include physical therapy, speech therapy, and occupational therapy. Almost all of our transitional care guests return home within a matter of days or weeks.

  • Independent Living

    Our independent living community is designed for active seniors who want a maintenance-free lifestyle with the freedom to come and go as they please. Residents enjoy private apartments, engaging social events, and convenient amenities—all without the worries of home upkeep.

  • Long-Term Care

    We are proud so many residents call Signature Pointe their permanent home. If you or a loved one are unable to live independently, we offer medically supervised long-term care. Our nurses and nurse assistants provide around-the-clock clinical care and assist with activities of daily living. Our long-term care residents enjoy a variety of social and wellness activities, recreational outings, and quality dining in a safe environment.

  • Memory Care

    We are a leading provider of memory care services in the community. Our purpose-built facility is designed to ease the burden a decline in cognitive function places on our residents and their families. Memory care at Signature Pointe provides a secure environment supplemented with enrichment activities to stimulate cognitive function and spark a lifetime of memories. We offer both Assisted Living Memory Care and Skilled Nursing Memory Care. This way, there is never a need to move to a higher level of care with your loved one.

  • Palliative & Hospice Care

    Palliative and hospice care are often provided in the home. But for many families, in-home hospice may not be the best solution. If you or your family have chosen to no longer actively treat a terminal or life-limiting illness and are considering a symptom management approach, our medical director and clinical team can assist with a referral to a best-in-class provider while you reside at Signature Pointe.

  • Respite Care

    Signature Pointe provides temporary respite care for families in both the memory care and the skilled nursing setting. Being a caregiver is a labor of love, but it can become overwhelming at times. Caregivers need breaks to emotionally recharge their batteries, to travel out of town, or to rest after their own illness or hospital stay. Regardless of the reason, you can have peace of mind knowing your loved one is well cared for until you are once again able to look after them at home.

I stayed at Signature Pointe from February16 to April 9 this year as a rehab patient recovering from a serious foot surgery. Had no idea what to expect from such a long stay in assisted living but I had to have this surgery. I can say not only was I well taken care of, I enjoyed my time there and it was a little hard to say goodbye to some people. Nurses, the physical therapy staff, administration… Everyone was great! Attitude in that place was awesome and it was cool how everyone on staff got along and worked together. I was there long enough to get to know some of the residents on the first floor. That was fun and they all seemed to be very happy there. My insurance sent Signature Pointe to me as one of several options. I live close so I stopped in one afternoon late January and Ronald was gracious enough to take me on a tour. I told my surgeon and insurance Signature Pointe is where we are going. I’m still doing PT at home but as soon as I’m back in shoes I want to stop in to tell everyone on the third floor thanks again. I want to say I even had a lot of fun in there. (Totally unexpected) Think I’ll check on some of my friends on the first floor. Rehab or assisted living, I would seriously look at Signature Pointe.

My mom needed Assisted Living so I looked for these things: her safety, good care, and an environment where she can be happy. Due to a fall mom needed Rehab care first and Signature Pointe is the only place I’ve seen that provides all levels of care. We spent a month in their Rehab and I got to see firsthand the level of medical care and custodial care that they provide and was very impressed. When released from Rehab we simply moved downstairs to Signature Pointe Assisted Living which was much less traumatic than changing places. The level of care is excellent and the food is pretty good. The staff is very friendly and helpful and it’s clear they truly care about what they do. They treat their residents with respect. Mom is kinda picky but she is very happy with Signature Pointe and everyone on the staff. She’s meeting other residents that say the same. The old saying is true: Mom’s happy so I’m happy! On-Site Pharmacy

Signature Pointe features an on-site pharmacy service. The service is facilitated by a computer-controlled dispensary stocked with over one hundred of the most commonly prescribed senior medications. The dispensary is connected to a remote pharmacist who fills prescriptions written by a resident’s medical doctor.
